Oral disease risk scale for in older adults treated with antihypertensive medications

Introduction: The prevention of oral diseases is a challenge in older adults treated with first-line antihypertensive medications.
Objective: To develop a scale to assess the risk of oral diseases due to first-line antihypertensive medication treatment in older adults.
Methods: A technological innovation study was carried out in the Stomatology service of the polyclinic 14 de Junio, in the Jobabo municipality, Las Tunas province, between October 2023 and February 2024. Two random samples were selected n1= 415 (cases) and n2= 830 (controls), leaving two controls per case. The first-line antihypertensive medications consumed (hydrochlorothiazide, captopril, enalapril maleate, nifedipine and amlodipine) were determined as independent variables and the presence of oral disease (dental caries, gingival enlargement and xerostomia) was determined as the dependent variable. The scale was derived from the binary logistic regression analysis of the identified antihypertensive medications.
Results: Treatment with nifedipine in older adults reached a score of 6; hydrochlorothiazide and captopril, 5 and enalapril and amlodipine, 3. The area under the curve was 0.65 with a 95% CI: 0.62-0.68, it does not contain 0.5 with a standard error of 0.01.
Conclusions: It was possible to develop a risk scale for the prevention of oral diseases in older adults due to treatment with first-line antihypertensive drugs, containing five medications that act as risk factors.
