Design, manufacture and use of a removable appliance for the treatment of dental transposition

Introduction: Dental transposition is a form of ectopia in which a permanent tooth develops and erupts in a position occupied by another permanent tooth. Treatment is complex and may include accepting the transposition and aligning the teeth in that position, extracting one of the transposed teeth, or orthodontic correction.
Objective: To describe the design, manufacture and use of a removable appliance to orthodontically correct the incomplete dental transposition of the upper canine with the lateral incisor in the initial stage of treatment.
Methods: A developmental research study was conducted at the Provincial Pediatric Hospital Eliseo Noel Caamaño, in Matanzas, consisting of two stages: propositional, and descriptive/prospective. A removable appliance was manufactured with double Adams type retainers for greater retention and anchoring; a pin spring with 0.7 mm steel wire, and an acrylic portion to join the metal elements. The force of the spring is 20g to achieve pure vestibular movement and move the canine away from the lateral root.
Results: A device of easy manufacture and management for the patient was obtained, which allowed the orthodontic correction of canines retained by vestibular and in incomplete dental transposition with the lateral incisor.
Conclusions: The designed device represents a technological innovation of easy manufacture and high clinical efficacy for the initial treatment of incomplete dental transposition of the upper canine with the lateral incisor. Its operational simplicity makes it a valuable tool to use in orthodontic services.
