Materials and methods: we used the methodological triangulation, focal groups and surveys with experts, users, formal and informal leaders, persons with heart disease risk factors and relatives.
Results: the model has a structure defining two dimensions and six categories interrelated in a dynamical way.
Conclusions: the feasibility and pertinence of the health promotion model, directed to the population with risk factors of ischemic heart disease in the primary health care, was showed.
