Endophthalmitis-complication of the glaucoma and cataract surgery

A study of five cases diagnosed as endophthalmitis is exposed. All of them were inpatients in the Ophthalmologic Ward of the Faustino Perez Hospital in Matanzas , from April to November 2004; two of them were men and three women.As common antecedent, they had ocular surgery: three cataract surgery with intraocular lens and two glaucoma surgery. The clinical picture began between five and ten days after the surgical intervention; the main symptoms were an intense ocular pain and the decrease of the visual acuity. As persistent signs: corneal infiltration and edema, hyppopion and lost of the red-orange color of the fondo de ojo.The treatment consisted in Vancomicine (systemic) as first choice; if there wasn't Vancomicine in the pharmacies Gentamicine + Cefalosporine was used. As local antibiotics, fortified eyewashes were applied. In all the cases, and excretion cultures were made with negative results. The evolution of the five cases was unfavorable, three with a spontaneous perforation and a bulbar atrophy that refused a surgery. The theme is reviewed, reconsidering Vancomicine as the first option. We insist in the importance of prevention and the deep study of the possible epidemical chain.Downloads
